If he has had surgery in the past, it is very possible there is scar tissue formed in his abdomen. This, if irritated somehow (overeating, overexertion, harder than usual BM's etc) can be very painful. It can also cause more blockages. I kept getting blockages after my 2nd surgery. Eventually I went in because of a severe obstruction again, had to have surgery again. Everything looked ok as far as the actual intestine (no crohns disease etc), but there were adhesions throughout my abdomen. They ended up cutting out a tremendous amount of scar tissue. Now my stomach hurts all the time. If I am even moderately formed (bowels), I am in extreme pain.
