Quick question about insurance in US

So during the summer I spent a night in a hospital in America. Where I'm from, I don't have to pay for anything. Anyway, a bill arrives for $2000+ which my own insurance company here sorted for me (I won't voice my opinion about how healthcare in America makes my eyes pop, or how people who can't afford to pay can afford to live...that's for another day!).

I got a letter today sent requesting $138 and it says my insurance paid the bill but this is what's left over. I'll be sending that to my insurance company to pay, but just wanted to know:

In the US, do any insurance companies pay the full cost of treatment/ drugs or is it always co-pay??

Insurances vary over here. Some have co pays or deductables that have to be paid by the patient/insurance holder & the balance of the bill paid to the provider. Other insurance companies pay things 100%

You could very well get a bill from the hospital and the a seperate bill from the doctor &/or radiologist. Many times the doctors are not "employed" by the hospital so they do their own billing.

I hope this explains some things to you. Do you get an "explanation of benefits" when your insurance pays a claim for you. I get one for each claim paid & it shows me what was billed, how much they paid & what my portion is, if any!
