I thought it was documented, but I would have to find it. Its an aeresol enzyme of sorts (for lack of better words) so IMHO it would make sense that it can aggrivate hemoptysis. The Vitamin K is added since we dont absorb vitamin K well. Vitamin K helps with blood clotting. How much you need depends on what your normal levels are and what your clotting time is. I personally use 10 mg tablets (up from 5 mg) taking 1 tablet twice daily. I take a couple at the time of an active bleed also.

Pulmozyme caused hemoptysis for me too. I never had bleeds, as a matter of fact, until I started Pulmozyme. It was determined that I could use it every 3rd day w/o bleeds, but I finally just quit it all together....too hard to keep track of which day was the 3rd day and I wasn't sure just how effective it was to use it twice a week anyway.

I've had bad hemoptysis for 3 years--enough that I had three embolizations. after the 3rd embolization I was still bleeding. Every day I would have several mouthfuls. I stopped Pulmozyme and have not had so much as a streak since. That was before Christmas. Pulmozyme definitely made me bleed.
It was hard for me the first week after I stopped because I felt so congested. However I have supplemented with Hypertonic Saline and oral N-acetyl cysteine. Last visit my pfts were actually higher. There is no way I would ever go on Pulmozyme again.
