Should the Vest make my daughter cough?

My daughter is fairly new to using the Vest. She has a HillRom model 105. She uses it on a standard group of settings that the nurse who trained us on the machine recommended. We do not vary it during her treatment.

It doesn't make her cough. Should it?

Note that my daughter has no baseline cough, her lungs always sound clear, she has no history of coughing up gunk, she's quite healthy... it may be that the vest isn't making her cough because she doesn't have enough mucus in there to be shaken loose?

Or does it indicate that we're not doing it right somehow?

Would love to hear other parents' experiences.

Chest pt or the vest has never made me cough, unless I was really sick. I have to make myself cough, and from the chest pt technique I've learned, the whole point of chest pt is to make yourself cough, and percussion or the vest helps loosen it so you can cough it up easier. This is harder with young kids, but learning the right way gets easier when you are older.

Hi.

My daughter does not have a baseline cough and does not have a productive cough on the vest as a norm. Once last summer she had a really bad cold and the vest would make her cough.

When we do the vest, we stop every 5-6 minutes and have her huff and cough. 99% of the time it is dry.

DS is like the above posters -- doesn't cough unless he's sick or coming down with something. Was the same with CPT, didn't cough with that either. We do pause every 5 minutes or so and have him cough.

Once DS got used to the standard vest settings, we did modify them -- increased pressure, varied the hz settings, have since increased the amount of time to 30 minutes per vest session...

I can't speak as a parent but I can speak as someone who has been using a vest since about 2004.

In terms of coughing my vesting is non-productive unless I use the nebulizer at the same time. Presently, I am using a 7% Hyper Sal. This combination induces coughing and is very productive.

Emily has always coughed some when doing the vest (but then again, she has a baseline cough too) but coughs MORE now that we have the vest programmed with Minnesota protocol. If you ask your clinic RT or PT about it you should be able to get more info about that. It varies the speeds and settings to most effectively target all areas of the lungs; small and large airways.

She may not have anything to cough up at this point, but I agree with the others that pausing every 5min or so to have her try to huff (if she can) and cough is important to start teaching her the objective of airway clearance.

eta> I also agree that using a neb while vesting makes Emily's sessions MUCH more productive. Nebbing albuterol is far more effective than even 4 puffs of Ventolin and the effectiveness of nebbing during vesting is a large part of why we introduced hypertonic saline.

My son and I both stop our vests every 5 minutes and huff cough. This is very important otherwise you are just shaking down anything in there.
