The only advice I can give you that isn't prayer related (and I will pray for you anyways <img src="i/expressions/face-icon-small-wink.gif" border="0"> ), is that when I have had my 2 abdominal surgeries as an adult (both incisions were about a foot long each, top to bottom- hate that pain!!!!) is to get a pillow, either a standard or travel pillow and hold it tight across your incision when you cough- it helps more than I thought it would.
Can you do home IVs? Have they done a sputum culture to make sure they are giving you the right antibiotics?
I had something very similar (for about 15 or more years), and turned out that I had a <i>few</i> things- I would get your doctor to check you for them, if he hasn't already (I had my gallbladder removed before all of this, fyi)-
~Undiagnosed pancreatisis
~H. Pyloria bacteria- causes ulcers, but can cause other problems and pain- I had no ulcers, but the chronic pain ceased after I was treated for it.
~A gallstone/bilestone in my common bile duct
~A year later, I had an infection in my common bile duct, due to sludge and "backwashing" from my small intestine into my bile duct/liver. Backwashing occured because of the the way they did my previous bile stone removal (scope)- they cut the spincter between the duct and the small intestine.
You may also be having a problem with adhesions- I've had 3 adhesion cleanouts- sucks, but...and, I have one that, when I get a stomach bug, closes shut, right below my stomach.
I'm dealing with this again this week, so you have my empathy on the tummy issues...
