Who Performs an IUI?

I am going to try to make my story as short as possible.
We got pg on our honeymoon. I was taking oral antibiotics at the time, not sure if they helped thin some cm but I got pg. That pregnancy ended in miscarriage, not CF related, just something that happens.
We started TTC again right away after the miscarriage. After 12 months of TTC with charting basal body temp, fertility signs, ovulation predictor kits and natural cm thinning agents we went back to my OB/GYN. She order a semen analysis and an HSG test. She also sent us to the fertility clinic.
We met with a Reproductive Endocrinologists and together we decided that as long as the semen analysis and HSG came back normal we would do IUI the following cycle. We decided to do natural IUI, no fertility drugs because I was ovulating on my own on a very regular basis.
Semen analysis and HSG were totally normal. We got pg on our own that month, I ovulated the day after the HSG test.

Our daughter was born 9/6/05

Fast forward to our daughters 1st birthday and we decided we are just going to see what happens so we are simply not going to avoid pregnancy. This goes on for a few months with no pregnancy. Every month I could feel when I was ovulating and timing was right but nothing. I started charting again and using OPK, still nothing. At the 6 month mark I contacted my OB. She ordered blood work just to make sure my hormone levels were o.k. and said to make an appointment with the fertility clinic when ever I was ready. I did not feel a really strong pressing need to get pg right away but after a stellar CF check up with the best pft numbers in years we decided to go for it. I called the clinic and made an appointment. Since we had been there before they got us in right away, no 4 month wait like the first time. We met with the RE and we had the same plan, natural IUI, no fertility drugs. We did IUI the following month and we got pg on the first attempt. I am now 25 weeks pg with out second child.

Because of my endometriosis, we did laporscopic surgery first, then 3 failed IUIs. When that didn't work we were advised to do IVF because of the endo. It back, 3 months post surgery. A surgery that takes normal endo symptoms away for 3-5 years. Mine was already visable on ultrasound 3 months later. So we got on the IVF schedule. It cost us $15K in cash, insurance didn't cover any of it (whereas they covered 50% of the IUIs, so our out of pocket expense for IUI was about $300, cheap compared to IVF!)

A surgery that takes normal endo symptoms away for 3-5 years. Mine was already visable on ultrasound 3 months later. So we got on the IVF schedule. It cost us $15K in cash, insurance didn't cover any of it (whereas they covered 50% of the IUIs, so our out of pocket expense for IUI was about $300, cheap compared to IVF!)

I went to a reproductive endocrinolgist, I was very happy with that choice. I had 6 unmedicated IUI's and then 2 medicated ( gonal-f) before I finally got pregnant. My RE is rather conservative, and really has the goal of a single healthy baby. Sh did not want to use clomid, she felt for me the risk of multiples was too high. She thought she could better control the number of folicles using injectibles. We agreed ahead of time that if there were more than 2 folicles the cycle would be cancelled. They used a lot of ultrasound monitoring. I have a healthy baby boy who was born on his due date. I have 2 girlfriend who do not have cf who also used the same RE. One had unexplained infertiltiy and took 4 years to have a baby( she has one now after using injectibles and IUI) the other has endometriosis and has boy/girl twins ( she used clomid and IUI)
