Why won't they listen??

ok here what happens when you do neb forms of drug that your pa is resistant to.

even though the pa is resistant to tobi when doing the neb it essentially overloads the pa strain and still works b/c the pa is overloaded with drug. also when placed on 2 drugs (cipro and tobi) it works on a synergy basis. ie the cipro weakens the pa bug which allows the tobi to work even better.

Inhaled TOBI is at a stronger dose than IV Tobra, since not as much goes to your kidneys causing toxicity. So, even though you may be resistant to Tobra, your PA may be sensitive to the stronger dose of TOBI.

Also, your PA can be sensitive to two drugs used together (even if it wasn't sensitive to one used by itself).

Although, TOBI is usually used on a month, off a month, some people like myself (although I think it's rare) have really bad reactions to TOBI. Thus, I only use it as needed.

I have Cepacia so I am resistant to almost every drug yet they still prescribe me the same combination when I need them. The fact is, whatever they end up putting me on does end up helping in the long run. Abx don't only fight the infection but they also help with things like inflammation. Certain combinations of drugs may help rather than one drug alone because as xosder22 said it works on a synergy basis. Resistance to the drug is only one factor in the combination. PLUS, if they put you on another drug that the pa is not resistant to, it may build up a resistance to it, so if one drug combination helps it is not worth exposing the pa to another drug to make your list of resistances even longer. Also, inhaled abx tend to work in that they overwhelm the bacteria so much that it is weakened. Maybe the combination of cipro and tobra will help together.

My peds doctor took me off inhaled tobi because i was resistant (he put me on colistin instead), but when i switched to the adult program, my new doctor put me back on tobi (and took me off colistin). he said even though my bacteria show resistant to tobi, it's such a concentrated dose when you inhale it that it is probably still effective.

i am about a week into the first 28 day course i've done in years (i've been very noncomplient) and it feels like it is helping a ton.

good luck with feeling better and getting your doctors to listen to you!
