Jennifer-I only skimmed through posts so forgive me for possibly repeating things.
If you live within1 hour of a infertility clinic then you can have him do the deposit at home and you can even drive it in. I have also known others who when giving samples for IUI's etc would get a hotel roomwithin an hour so as to avoid the clinic.
We went in for our infertility work up after abotu same scenario as you and there is hope! They ordered an SA for DH but he didnt want to do it and it was so far from where he worked and couldnt get time off PLUS we were POSITIVE it was me <img src="i/expressions/face-icon-small-wink.gif" border="0"> anyway after months of failed drugs etc he did go in and get the SA and low and behold we wasted all those months doing drugs for no reason as there was still barely a chance at all we woudl get pregnant with his sprem and we immediately had to do IVF.
Dont be to hard on him as it is very intimidating for them two fold one doing it in 'public' nasty place - my dh woudlnt even sit in chairs or touch things in room it felt too 'dirty' anyway and 2ndly that if the test shows bad they feel de-masculinated.
Dont be too hard on yourself either as a fertility doc wont even look at you until after a year becuase it is very common if everythign 100% perfect to take a while.
the test to clear the fallopian tubes just puts dye through them but for whatever reason alot women get pg after this test on their own even when it appears tubes already open-it's called a sonohystogram and it is a horrid test!
I agree dont call insurance look up in your plan on a website or have somene else with your plan call if you knwo someone as sometimes they will cover sono's or u/s until they find out for IF so sometime sit will slide through.
