I've had the same problem with Tobi and recently had to stop taking it. I've heard from my doctors that a lot of patients react badly the first time they use the drug, but others can use it for years (like us) and then all of a sudden they have bronchospasms which causes the SOB, tightness, pain..etc. I switched to inhaled Ceftaz, which smells and tastes terrible and I notice pain with that as well. It is weird though because Tobramyacin IV's are still working fine.
Have you tried Xopenex? I have a Xopenex inhaler that I used when I feel short of breath. It's different than Advair so you can do it more than twice a day. Really, I think you can do it as much as you need to throughout the day. Xopenex is basically a form of albuterol, but I shake and have heart palpitations with albuterol so I use Xopenex.
