Nasal Polyps

My daughter had FESS, Functional Endoscopic Sinus Surgery, bronchoscopy, and endoscopy this past Monday. We were told that her Sinus CT showed her sinuses were impacted by old sinus infections and that they needed to be cleaned out. After the sinus surgery the surgeon told us there was no old infection just Nasal Polyps. She told us that they are caused by allergies.

My daughter has been tested for allergies with blood work and scratch tests. Her bloodwork shows low normal IGE level and <.1 on all tested allergies with .3 to .6 being unknown clinical significance and .6 and up being an allergy. Are there any unusual allergies she should be tested for?

Do any of you have you have nasal polyps and no allergies? If so what causes the polyps?

Any advice would be appreciated.

Thank you

I may can help there. My Abby has had 15 surgeries in the last 2 years, 9 of which were the Endo. Sinus Surgeries. She has been allergy tested twice at 2 different places and came back with 0 allergies both times. We have learned that the polyps are from her sinus' getting irritated by the particals that she is breathing in, which is causing inflammation, the inflammation in turn causes sinus polyps. In our sweeties case the polyps or the inflammation(we're not really sure) ate the bone between her sinus cavities and her eye cavities on both sides, so when the surgeon removed the polyps (that were holding her eyes in place and we had no idea) instead of the bone, her eyes fell back in her sinus cavities. She has had several eye surgeries to correct this problem and is a very happy healthy little girl, and this is not the norm!! I must stress that. Abby had to have her sinus polyps removed every 3 months, because they were out of control.

We since bought the IQAir whole house HEPA Filter and for 1 year she had 0 sinus polyp surgeries!!! YEAH!!! Now we have sold our trailer house and have moved in with my mom while our home is being built and she has started back up again But I have a feeling that as soon as we are able to get in our new home with the HEPA filter and UV lights in the ducts she will once again be good.

I am not trying to scare you but I sure wish I would have known to watch for these things before it happened to us...things might have been much different for my pumpkin. Most people that I have seen and heard about have to have polyps removed every 3 years, not every 3 months. Abby is on singular, zyrtec, and veramyst nasal spray(which is a steroid to help keep the swelling down), and sinus rinses. They told us to absolutely stay away from any kind of smoke or dust. I'm not sure how old your little one is but Abby started having these when she was almost 4 and she did fantastic with the surgeries. They didn't do any packing and it dripped blood out of her nose for about probably half a day or a day and she through it all bounced around like nothing had happened(even a few hours after the surgery); with me chasing her telling her you have to be still for mom. Before her surgery she could not breathe at all through her nose, afterwards was wonderful!!! Sorry to talk your ear off, if I think of anything that I may have forgotten I'll repost or if you have any questions feel free to ask. Good luck! Our kids are stronger then we can ever imagine!

I had 1 sinus surgery for polyps when I was a baby, and used a nasal spray for years after that and had no problems. Then at about 14 years of age they came back and I had 5 or 6 surgeries to remove them over the next few years. I haven't had them since (I'm 26 now). So not exactly sure what causes them, but I have no allergies, and had always been told that it's a consequence of "sinus CF". I think sinus infections themselves might cause polyps to pop up (since infection causes inflammation), and irritation from any inhaled particulates would make that worse. I'm not a doctor, but that's my best guess!

I come from a medical background (Medical assistant / Paramedic) with 17 years working in an ENT/Allergy office. To be honest the only person with CF that I have ever tested was my son when he was about 13 years of age at the time (he is 15 now). He was negative to everything. I used a prick method and its usually fairly accurate, although I prefer to use Intradermal Skin End Point Titration as I get better results.

With that being said, every single non CF person I have tested that HAS positive history of nasal polyps showing on a CT scan have been allergic to several fungal allergens. There is definitely a link with the non CF patients between molds/fungus and polyposis. Not to contradict myself here, but I have thousands of other patients that have tested positive to molds and are negative for polyps. Could just depend on the patient. But more than likely it has more to do with the over production of mucus in the CF patient and forming polyps in the sinuses.

My son had a polypectomy when he was around 6 years of age. Repeat CT-sinus last year shows everything under control.

Hope any of this helps.

EDIT: as a side note, you might have your doctor consider using Nasonex nasal spray. Its a steroid that is used to treat allergies as well as help shrink polyps. My son uses this daily (when I can remember to remind him to take it daily lol)
JennyC and dsheldon,
Thanks for your responses. We added a hospital grade filter to our heat and air system this past winter when we had to replace both due to age/inefficiency. We have virtually no dust with the new system, so hopefully they won't grow back again. She was put on Flonase to try to prevent the swelling from returning. She took Zyrtex for as long as I can remember and she is now 12. We stopped the Zyrtex last year as with or without it it made no difference to her. As for sinus rinse she did them well the first 2 days after her surgery and now she says it burns so much she cannot stand it. I am not sure if she just tolerated it while taking the high does pain med and we should go back to using it so she can do the rinses or just give her a day and try again with the rinses.

JennyC what UV lights are talking about? Where do I get them?

Thanks
Jen

The UV lights are going to be new for us but our AC guy bought and installed them for us, it is suppose to kill 98% bacteria. It was very affordable also. It costs us about $50 for purchase and installation. I'm can't remember how often he said that we would have to replace the bulbs but they were like $4.00, if I am remembering right.

Thanks for your response. We recently did blookwork for mold/fungus allergens because my daughter was very opposed to skin testing. She tested at <.1 for everything so the Dr. said no need to test further. The lab work says .3 to .6 was incolnclusive and .6 and up was an allergy. So I think <.1 meant nothing measured at all. We have a follow-up with the allergist next week. The polyps caught us off guard as we thought her sinuses were impacted with old infection, but none was found only the polyps. I will specifically ask for the those allergy tests if the doctor thinks it is as all possible given her blood test result. Thanks for your insight. My daughter is in limbo for CF diagnosis. This seems one more thing leading us back to the CF possibility.
