number of enzymes

Our nutritionist gave us the maximum amount of enzymes that our ds can have (not sure what they base it on...maybe weight/size). Anyway, he can have a max of 5-6 Creon 10 per meal. We usually give him 4. He was needing about 1 Creon 10 per 10 grams of fat he ate (to keep his stools non greasy) but now he needs 1 Creon 10 for about every 8 grams of fat. We try to get him a minimum of 20 grams of fat and 15 grams of protein per meal.

Tom,
Thanks for the information about the enzymes. I have never heard about the other two types of enzymes that digest proteins and carbs. Thanks!!
Sharon, mom of Sophia, 7 and Jack, 5 both with cf, Grant, 5 months no cf

I've seen a pretty wide range of enzyme 'dosing' amongst the kids here, but that seems appropriate to me, since CF is such a variable disease. Some kids don't have many digestive issues so might only need a very low dose of enzymes, others need high doses and still can't keep weight on.

Sullihs, I like your nutritionist's approach- it's intuitive and lets you follow your son's symptoms as a guide. What's coming OUT is a pretty good guide as to what you need to put in (number of enzymes), more so than an arbitrary number, I think. I am sure they would let you know if his growth becomes a concern as well.

Since enzymes come from pigs, the relative amounts of the three types of enzymes is way off from what humans need. The amount of Lipase (for fats) is extremely high compared to the other two. That's why a CF nutritionist might tell you to only worry about the fats. If you give enough Creon pills for the amount of fat, you will have enough enzymes for everything else. Also, Proteins and Carbs are easier to digest and so small amounts may not need any enzymes (though to be safe we almost always give one Creon 5 for anything our dd eats.)
