Pulmozyme Question

Emily does Albuterol (by inhaler, before she vests), then Pulmozyme (while she vests), then Flovent (inhaler, when she's done). If she's sick or congested it's Albuteral, Pulmozyme & Pulmacort (neb)... both nebs done while she vests. This was the order that was suggested to us by her dr.

She does not do Tobi but I know that they instruct us to do inhaled abx last at our clinic, after all other meds and after vest/cpt ...this being stressed during the session about airway clearance at our last education day.

eta> as Tonya mentions below, there is an ideal 'wait' period for Albuterol to start working. I try to give Emily's Ventolin a bit before she starts her vest because I do notice a difference in how much she coughs if she has a little wait time between doing her inhaler and starting the vest. However, since it's an inhaler and not a more time-consuming neb, it's easy to at least have her *do* something with that wait time; i.e. get dressed & brush her hair in the AM or put pj's on at night... since time is always SUCH an issue!

Wow I guess we are doing it all wrong then....we do albuterol inhaler then cpt then pulmozyme...so I guess we need to do inhaler then pulmozyme then wait and then do cpt last? I'll make sure to ask this question at Camerons Tuesday appt!

Ideally you would use your brochodilator (albuterol) first, then hypertonic saline, then pulmozyme, then vest or cpt, then any inhaled abx (TOBI). Basically, you want to put the inhaled abx in nice clean lungs, instead of on top of mucus that will be coughed out and leave the body. Unfortunately, like a previous poster stated, time is not a friend. When we have time, Andrew does everything seperately. However, when we are in a hurry,(before school) I have him do his vest after HTS and during Pulmozyme. I have also been told that we should "ideally" give him his albuterol 15 minutes before we start his nebs, too. I am so glad we all understand each other! Where else can you go to find friends who can understand an hour and half routine of meds, morning AND night!?...just to stay healthy enough to not "look" sick!?
