Emily does Albuterol (by inhaler, before she vests), then Pulmozyme (while she vests), then Flovent (inhaler, when she's done). If she's sick or congested it's Albuteral, Pulmozyme & Pulmacort (neb)... both nebs done while she vests. This was the order that was suggested to us by her dr.
She does not do Tobi but I know that they instruct us to do inhaled abx last at our clinic, after all other meds and after vest/cpt ...this being stressed during the session about airway clearance at our last education day.
eta> as Tonya mentions below, there is an ideal 'wait' period for Albuterol to start working. I try to give Emily's Ventolin a bit before she starts her vest because I do notice a difference in how much she coughs if she has a little wait time between doing her inhaler and starting the vest. However, since it's an inhaler and not a more time-consuming neb, it's easy to at least have her *do* something with that wait time; i.e. get dressed & brush her hair in the AM or put pj's on at night... since time is always SUCH an issue!
