I almost never go braless. I am fairly moderate sized 34D ( but can also wear a 36C). I always wear the underwires because nothing else seems to offer support. Now while i am sick i tend to not wear a bra ( only at home)because it feels so restrictive. If i have to go somewhere i find one thats a bit stretched out and i wear that one because it isnt too binding.
I would suggest a bra that has a lot of soft elastic that can stretch so you can feel more comfortable.

For when you are well, a normal bra is fine if it fits properly... maybe you guys are wearing ones that are too tight for you, if they constrict your chest. Go to Victoria's Secret. A little pricier than stuff in other stores but they are very good quality, last forever, and the majority of them are amazingly comfortable.

One thing to note is that your chest size (the number size) might creep up as you get older and get more barrel chested. I used to wear a 32 or 34, now I wear a 36 bare minimum. Keep that in mind when you are shopping and don't be afraid to get a bigger size or measure yourself before you go shopping.

I live in the cotton light support from the Gap Body, they are not too expensive and VERY comfy - I even sleep in them (no hubby does not appreciate it but the girls do). Good luck with this delima...

I noticed this pain under my shoulder blade was much, much less severe at night in the couple hours after I "let 'em loose" and in the mornings before I "reign 'em in." lol However, I am very small in that area, and therefore I never connected it to the bra until I read this post. Just like I didn't realize the area was sensitive to pressure until the dr pushed directly on it. Now I realize that the back strap sits right on that chest tube scar. Maybe I could take it off a bit earlier at night or something. lol
