I was on TPN when I was pregnant with my son (about 6 years ago) and it worked wonders! 7 months into the pregnancy I was 10 lbs less than my starting weight...so my doctor decided to put me on TPN. I gained 20 lbs in 2 months! It worked really well! Although I don't have CFRD....but they really monitor your labs when you have TPN-I had to have blood taken every week, they would check my vitamin levels, etc...and formulate the TPN for that week based on what my body needed. It really was amazing. My TPN was administered through my PICC line and it took 12 hours, we would start it at 7pm and be done at 7am. I was on it for 2 months and had no liver problems.

My son was on tpn for his first two weeks of life. He had high liver enzymes for a year after that. Although his liver enzymes have been good for 3 years since and his liver is the right size he still has the "cf liver disease" diagnosis. His GI dr said the tpn after his birth was really hard on his liver. I would definitely go for the g-tube. I would think it is much safer and overall easier. My daughter got a g-tube 8 months ago. She was 36 pounds when she got the g-tube and she is now 48. That is 12 pounds! Yea! I don't really understand how you are not ready for g-tube but you said your weight is dangerously low. That is what the g-tube if for. The g-tube is not permanent. It can be easily removed if you don't need it.
